How to register with us …

Bring in your medical card. If you cannot find it, speak to one of our reception staff who will give you a Registration Form (GMS1) and a New Patient Medical Questionnaire to complete.

We will check it and, if we agree to accept you as a patient (e.g. if you live within the area we cover) we shall then arrange for you to have a new patient health check which involves a 20 minute appointment, we would like you to bring a small urine sample in a clean, sealed bottle.

Your registration form will be sent to the Health Authority and you will receive confirmation that you are registered with us.

How to obtain Test Results …

Please telephone the surgery between 12.30 and 13.00 and ask to speak to a Practice Nurse.

How to order Repeat Prescriptions …

Please do not telephone the surgery to ask for Repeat Prescriptions. Repeat Prescriptions can be ordered either by completing a 'request' slip at the surgery or by email to:
Greenwood.Meds@nhs.net. If on your current Repeat List there are no more issues showing, you will have to complete a medication before your next request can be issued.

How to obtain Holiday Travel advice …

Download the form on the link below and complete the questions and return it to the Surgery. The Practice Nurse will check the details and advise on any immunisations or medication recommended. We then ask you to contact the surgery after 2 days of us receiving the completed form to inform you of the next step.

How to request a Home Visit …

Home visits are strictly for people who genuinely cannot get to the Surgery. Please telephone the Surgery between 9am and 10am whenever possible to ask the receptionist for a Home Visit.

How to make a complaint or comment about us …

If you should wish to make a complaint or comment on the services you receive from this Practice … please write to Dr. Cormack.

You will receive an acknowledgment within 7 days, followed by a full reply (providing we are not kept waiting for other information e.g. a hospital report or witness statements) within 14 days. If you are not satisfied with the reply you should let us know – we will undertake to pass your complaint to the appropriate authority.
